# BUG-000021 first completed VPS field case
Final input: operator-supplied `BUG-000021-Fallakte.zip`, revision 2, generation 123, OUTPUT_READY/outbox. This analysis read the package without changing it or invoking a model. Earlier blocked snapshots remain separate. The independent usage snapshot is `token-usage/bug21-final-g00000123-20261024.json`.

The explicit operator grant was consumed by entity assessment attempt 4. Its model response repaired only the localized search, changing no_match to ambiguous. The complete base-name identification of Furien (35286) was retained unchanged. Joint planning then encountered a recorded LLM_TIMEOUT/LLM_UNAVAILABLE and resumed successfully in round 2. Thus the apparent period without phase progress has a recorded provider-timeout/retry explanation; it is not evidence of a deadlocked worker.

Research produced six evidence records. The creature relationship tool found a single spawn, GUID 367807, on map 1, zone 405, area 2404, with no game-event link. This refutes the initial missing-spawn hypothesis within the query scope. An attempted startup query requested unverified column `phaseId` and was rejected as PROJECT_DATABASE_COLUMN_UNKNOWN; a schema query followed. The subsequent row query retrieved PhaseId 169, PhaseGroup 0, phaseUseFlags 0, phaseMask 1 and coordinates (189.557,1770.96,91.2064). Other queries obtained creature_questender and conditions schemas and discovered phase table names. They did not retrieve questender row contents, phase definitions or character phase membership.

The model selected H2 (phasing) and requested early diagnosis. The final diagnosis accepted this, rejected H1 and left H3 unresolved. It produced an assigned SQL migration-template coding request using {{TARGET_PHASE_ID}}, together with a human-dev request to verify intended phasing and questender relations. No executable patch was generated or tested. Final output validation status is valid, which establishes contract conformance, not factual correctness of the root-cause conclusion.

Quality assessment: the phasing lead is plausible and backed by actual spawn data. However, the diagnosis claims the reproduction demonstrates that the character was not in phase 169. No character phase-membership observation was located in the supplied reproduction/case records; the research queries did not establish it either. The claim should be treated as an inference, not a measured fact. Likewise the relevant core phasing semantics and intended quest progression were not investigated. The supported suspicion is useful, but the confirmed label and assertions that alternatives were isolated are stronger than the reviewed evidence supports. The coding request allows only an NPC PhaseId update even though the diagnosis explicitly leaves open that the quest should grant the existing phase; human verification is therefore material to the selected repair direction, not merely selection of a numeric placeholder. No program behavior was changed on the basis of this single case.

Operator release to completion: 836.891499 seconds (13 minutes 56.9 seconds), excluding prior manual downtime. Revision-2 recorded model time totals 240.523818 seconds; timeout usage/time is not available in metadata. Revision-2 measured usage: 190037 input, 28659 output including reasoning. Clean usage retaining first regular calls: 116270 input, 27488 output. First metered joint-planning call is classified regular even though its stored label says correction. Entire case including failed revision 1: 268661 input and 42049 output, plus one unmetered timeout. Do not add earlier snapshots to these cumulative totals.

Costs use the saved USD price coefficients, with input/cache ambiguity explicitly bounded. Revision-2 known-usage range is USD 0.129899625–0.207924225; clean range USD 0.116366625–0.178580025. These ranges exclude any unmetered timeout charges and unknown storage, and are not EUR billing totals. No current provider-account balance was supplied. See the JSON for per-call and per-phase estimates and unknown values.
